Initial Consult

Monthly Memberships:

All memberships include monthly visit fees, unlimited messaging with your practitioner, nutrition and health coaching.

Option 1:

Membership without Insurance (Compounded Medication)

~ Injectable Medications: $279-499/month* (depending on medication selected/dosage)

~ Oral medications: $109-349/month* (depending on medication selected/dosage)

^{*Includes medication from compound pharmacy, any supplies needed (e.g. needles for injectable medications). Shipping costs are extra.}

Option 2:

Membership with Insurance-Covered Medication: $99/month**

^{**Cost of medication is separate. Membership includes a prescription sent to your preferred pharmacy (medication cost determined by pharmacy/insurance and paid directly to pharmacy). There is a one-time $49 fee if Prior Authorization needs to be submitted to insurance for medication approval.}

Extended Follow up visit (45 minutes): $119

Routine Follow up visit (30 minutes): $99

Brief Follow up visit (15 minutes): $65
